The PAW Center works alongside your Veterinarian to get your pet feeling their best. Clients of Mad River Vet Hospital do not need to have their veterinarian submit a referral form as Mad River Vet Hospital and the PAW Center share a record system.
If you are a DVM and would like to submit a referral you may contact us at pawcenterrehab@gmail.com for a paper form, or you may complete the online form below. The PAW Center understands that you remain the primary care provider. PAW is only offering rehabilitation and conditioning services.
